LifeNet Health provides a comprehensive benefits program to all benefit eligible full-time and part-time employees. You are eligible for benefits if you
are a regular full-time employee working 36+ hours per week or a part-time employee working 20-35 hours per week. Benefits, unless stated otherwise, will
be available on the first of the month following 30 days of employment. Your eligible dependents include your legally married spouse and children up to age 26.
Company Paid Benefits
The following benefits are provided by LifeNet Health at no cost to you.
Basic Life and AD&D Insurance
- 2x your annual compensation
- 2x annual compensation minimum benefit / $700,000 maximum benefit
Short-Term Disability Insurance
- Covers 66 2/3% of weekly base salary

Benefit begins on the 11th workday of injury or illness and continues until the earlier of recovery or the end of the 90th calendar
day for an approved disability leave
Long-Term Disability Insurance
- Covers 60% of basic monthly earnings up to a $10,000 monthly maximum benefit
- Benefit begins after 90 days of disability and continues to the earlier of recovery or age 65
- Coordinated with income from other sources (e.g., Social Security)
Pension Retirement Plan
Contributions made by LifeNet Health only and you are eligible for this plan if you:
- Attained the age of twenty and one-half
- Completed six consecutive months of service and work 1,000 hours
- Employed by LifeNet Health on the last day of the plan year, December 31st

The EAP can help you find solutions for everyday challenges of work and home, along with more serious issues affecting emotional and physical well-being.
Tuition Reimbursement Program
Provides reimbursement of covered costs for full-time, part-time, and Per Diem employees pursuing higher education in order to improve job skills and performance.

403(b) Plan
You are eligible to participate in the 403(b) Plan the first of the month after 30 days of employment. Through this plan you can
save for your retirement by contributing up to 100% of your eligible income until you reach the annual limit imposed by the Internal
Revenue Service ($17,000 in 2012). The 403(b) Plan is an employee only contribution plan.
